Message used for anything once the tensioner has been adjusted to the correct gap and the hold down fasteners are tightened for the last time. Yes, it's possible it caused this issue but my bet is on the washer.

You'll need to pull it all apart to inspect and know for sure. Looks like a new belt is in your future too.
