Running a little rich, maybe a little extra oil burning. I don't see any obvious HG failure, but its pretty hard to pick out unless its complete destruction. More than likely it was on the head side of the gasket, as the aluminum expands/contracts more. If I had to pick a bad spot out of that cyl. 5, I would look closer at the material between the cylinder and the forward coolant passage
