Last Update here. After thinking I had it nailed down to the ECU, I sent it off to DVDBURN for repair. Unfortunately, he bench tested the unit and said it was in working order. On the one hand great, ECU is good, on the other, still stuck figuring out the issue. Appeared to come back to a ground issue, and he gave me some pointers/info to help troubleshoot. I had checked the engine bay grounds multiple times, so a bit deflating to hear. He also mentioned THIS post in the FAQ, and that's what did the trick. No idea how I missed this in all my searches. I never realized the ECU was further grounded by being bolted into place. Since I had installed a new EFI harness, I never bolted the ECU back up, and just left it on the floorboard. As soon as I did that I was able to get the proper #6 ignition signal and no more code 21! A huge thanks to Gary/DVDBURN. He was very helpful and great guy to deal with.
