a close inspection of the carrier bearing. grab the ds and see if you can shake it side to side, and judge how much play rubber insulator around the carrier bearing has. they usually, in my experience, get a little soft and cause effects like yours, then all the sudden you make a hard shift and the insulator just completely rips apart making the car nearly un driveable.
