Message	Kinda extreme but the fuel rails and the injectors seem to be the only constants in your troubleshooting. Awhile ago I modified a set of rails for AN fittings and I was amazed how crappy the stock fuel rail's internal flow paths are, plenty of places for junk to hide and cause problems like you are describing. If you're using old style fuel rails, you can have mine for free.
