Message inertia change at the end of each stroke, but it's offset by the other pistons which are not changing direction (midstroke) at that instant. Think of it like this: Neglecting friction, if you were to spin your crank/rod/piston assembly inside your block, would it come to a stop by itself? Nope.

Weight of components has no direct effect on constant-speed operation aside from friction issues that arise from increased normal loads.
