To qualify for and receive your rebate check by mail, you must:
1. Purchase 4 qualifying KYB shocks or struts on a single retail parts purchase or a parts & installation invoice between March 1, 2014 and April 30, 2014. (Qualifying products include: KYB Excel-G, GR-2, Gas-a-Just, MonoMax, Strut-Plus, AGX, OE Performance and Performance Plus shocks and struts)

2. Complete and mail this rebate form along with the qualifying retail parts purchase or parts & installation invoice (copy or original) to the address on the front of this form. Invoice/receipt must include valid KYB part numbers.
All qualifying KYB purchases must be in a single transaction and appear on the same receipt or invoice. Submissions must be postmarked no later than May 31, 2014. Only one $40 for 4 rebate check per person, household and/or household address. Commercial addresses are not eligible. Offer void where prohibited, taxed, licensed or restricted. Cannot be combined with any other offer. KYB Americas Corporation is not responsible for late, lost, or misdirected mail. Submissions postmarked after May 31, 2014 will not be honored. Rebate checks will not be issued for incomplete or illegible rebate certificates, certificates without a qualifying receipt, or duplicate submissions. All submitted materials will become property of KYB Americas Corporation and will not be returned. Please allow 8-10 weeks for processing.
