Message I've seen pics and videos of wet systems causing puddling in the intake stream and then igniting, blowing the plenum off the engine. He's lucky that didn't happen, but I believe that something similar happened here.

I'm not saying it couldn't have come from the combustion chambers, but I feel it's more likely that the phenomena I'm describing is the culprit.
