The grain structure doesn't change. What it does is it gets all of the atoms moving at the same, appropriate, speed. This is a material stabilizing process that gets a material to its originally intended state where expansion and contraction thru heat cycles is minimized and the material is at its strongest point. The same process can be performed with heat, but I believe better results are achieved with the cold. Castings do have a relative grain structure, it's just not in the same way as rolled or forged materials. In fact there are a number of tools steels and alot of aluminum plate stock that is cast as opposed to rolled. If you're building a structural component out of aluminum you have to be aware of what spec to order to make sure and get the stronger cold finished material.
