Not true, more than once over the last 2+ years I have deposited checks/money orders for well in excess of $10k and never once had to complete any paperwork like that.... In any event, the thing to do is photocopy the check and the envelope, mail to the sender with a note asking what it is for. Wait 2-4 weeks. If you still want to try and cash, take to your bank, tell them to not deposit funds until they can confirm it has cleared. Of course this is NOT the honest thing to do as it is either A. a mistake (not likely) or B. a scam...
