on my injectors when I installed them in 8/02. They weren't gasoline resistant, and the cylinders got washed down with gas after about 45 miles. I am guessing that #3 got more gas in it than the rest, and the rod bent trying to compress it. The piston then was unable to travel all the way to the top of the cylinder (which was giving me my wonderful 87 psi compression in that cylinder), and was going lower, where it hit the oil squirter and broke it. That's my best guess, at any rate.
