Back when I was relocating my PCV hoses during the plenum pull, I bought a ton of hardware from McMaster Carr. I have several sets of these NPT barbed fittings and HUNDREDS of high temperature vaccuum caps, ideal for blocking off the OEM PCV fittings on the driver side exhaust valve cover.
These "kits" consist of two 1/4" NPT barbed fittings that will accept a 5/8" ID hose as well as two 3/4" vaccum caps. All you'll need to do is drill and tap your intake valve covers, install the fittings and run some hose to your PCV valves. This will require some extra fabbing to mate up the 5/8" hose from the barbed fittings to the PCVs, but that's up to you. See Woody75's write-up for more info.
